If you enroll in Loomis’ employee paid life insurance or disability plans after your initial enrollment period ends, or you choose to increase your elected life insurance amount,
you must provide a Statement of Health (SOH) to be approved for your coverage. After you enroll, you will receive an email to begin the process online.
